| Phase 5: Development and Implementation of Community Intervention Strategy |
During phase 5, program participants are exposed to a range of intervention strategies that address the priority issues identified during earlier phases. They identify program models with transferable content and program components that are adaptable to their own circumstances. Participants identify the resources required for implementation, develop an implementation plan, and initiate the intervention strategy.
Objectives
- Observe the range of intervention strategies/services implemented around similar health priorities in the United States and in the larger region of the local community
- Identify program models with transferable content/program components.
- Assess strengths and weaknesses of the community to implement and sustain the intervention strategy.
- Identify resources needed for implementation.
- Develop an implementation plan for review and discussion with the community steering committee.
- Identify other consultation needs, i.e., advocacy and policy development, media involvement and fundraising strategies.
- Implement intervention strategies.
Expected Outcomes:
- Identification, selection and implementation of intervention strategies and program models that are transferable and can meet the community needs
