In the third phase of the Healthy Communities process, participants organize an initial committee meeting to review available population-based data and identify the community's perception of its priority health needs and problems. This process involves multiple sectors of the community, mobilizing local leaders to establish focus groups and conduct a community survey.

Objectives
  1. Community leaders organize their first Healthy Communities committee meeting
  2. Community leaders identify community perceptions of health needs and community problems
  3. Available population-based data is reviewed
  4. The partners provide on-going consultation to community leaders

Expected Outcomes:

  1. Multiple sectors of the community are involved in the planning process
  2. Community committees are organized
  3. Community leaders agree to organize focus groups and a community survey
  4. Other resources are mobilized for action
