| Phase 2: Partnership Building, Planning and Tailoring the Process |
During this second phase of the Healthy Communities process, program participants visit model community health projects in the United States for consultation and observation. They continue training in community health analysis, including identification of key stakeholders, consensus building, use of existing resources, and the role of change agents. This phase also includes consultation with community members and key professionals in community planning, epidemiology, and health education.
During this phase of the program, the following objectives should be met:
- Expose local leaders to model Healthy Communities projects in the region and in the United States
- Continue training on community health analysis and mobilizing communities for change, including the identification of key stakeholders, consensus building, use of existing resources, and the role of change agents
- Provide consultation with community members and key professionals in community planning and health promotion
- Develop a work plan for change
Expected Outcomes:
- Competencies are developed in community health analysis, group process, organizational development, and community planning
- A preliminary step-by-step work plan is available as a planning resource guide
- Community leaders are confident in their ability to implement change
